On April 21, 2018 a routine sample tested positive, absent for E-coli. Repeat sampling wasn't conducted, a level one assessment was triggered. On May 2nd a routine, upstream and downstream samples were taken, all samples came back positive TC and absent for E-coli. On May 21st the level one assessment was submitted to ADEQ. On June 1st a routine sample was taken and submitted to the lab that came back positive for TC but absent for E-coli. On June 3rd repeat sampling was conducted sent to the lab within 24 hours. June 5th the lab results returned as RTCR001 TC present, E-coli absent, RTCR001UP TC and E-coli present, RTCR001DN TC and E-coli Present. June 5th the boil water advisory was issued. The well was disinfected using NSF approved product called "Well Safe" tablets, the storage tank was disinfected with NSF approve bleach called "Pure Bright" to a level of 1.0. The water systems' distribution lines were flushed using the storage tanks water of 1.0ppm, resulting in the distribution lines not properly disinfected. The system has triggered two additional level 2 assessments, the first in July and again in August.
Level 2 assessment conducted on September 17, 2018 by ADEQ inspector Steven Vogel, 16 hose bib anti-siphon devices have been installed at all of the trailer hose spigot hook-ups, a well raw water sampling tap has been installed, chlorination was added at the well site and a well slab had also been poured and sealed. The anti-siphon device at the RTCR001 routine sample site was not removed before the sample was taken making the sterilization of the spigot impossible. |
