OT SIG ISSUE DESCRIPTION: Routine sample was taken on 05/25/2021. Result was total coliform positive / E coli negative (TC+ / EC-). Repeat and triggered GWR samples were taken on 06/10/2021. The RTCR001UP result was TC+/EC-. All other results were TC-/EC-. Repeated turnover of management and maintenance personnel has resulted in inconsistent chlorination/maintenance of system. Discussions with higher HQ POC requesting that the importance of water system requirements be addressed as part of basic orientation of new hires for these positions. CORRECTIVE ACTION(S): The system was treated with Sodium Hypochlorite 12.5% - LIQUICHLOR (EPA No: 10897-26) solution for disinfection. No further action is deemed necessary. 06-10-2021 06-10-2021
