OT SIG On 5/23/2019 system's routine total coliform sample tested positive for TC+, negative for E-COLI. System's repeat samples taken 6/2/2019 all tested positive for TC+, negative for E-COLI. No record of steps taken to resolve the TC+ positive test. On 9/26/2019 systems routine total coliform sample tested positive for TC+, negative for E-COLI, no record of repeat samples were taken. On 10/28/2019 system operator conducted a level 2 assessment, cause of the positive test was not identified. System operator disinfected the system lines with Sodium Hydrochloride (12.5%). 11/24/2019 system's routine total coliform sample tested positive for TC+, negative for E-COLI, no record of repeat samples were taken. On 12/27/2019 second level 2 assessment was conducted by the system's operator, cause of the positive test was not identified, system reinstalled the chlorinator pump. On 12/30/2019 routine sample and a raw water sample, taken at the wellhead tested negative for for Total Coliform and E-COLI. On 3/3/2020 system's routine total coliform sample tested positive for TC+, negative for E-COLI. ADEQ Compliance Assistance Coordinator request a drinking water inspector to conduct a file review and a level 2 assessment. On 4/3/2020 inspector S. Vogel and Manager J. Bobko visit system and conducts the level 2 assessment. Routine sample location was found to be next to the food kitchen sink including a lever type faucet and faucet was loose from the sinks body. Hatch on storage tank was missing the required hatch seal. Chlorine level was above the colorimeter upper limit, bottle water was added for dilution, reading was still above the limit of the colorimeter. 04-03-2020 04-03-2020
