OT SIG Multiple issues identified during assessment: Well 1 needs casing that extends 12 inches above well slab with a screened air vent that is 1 foot above the well's casing and turns down 90%. Install raw water sampling tap for Well 1, remove soil run-on from well slab and dig around well slab to prevent further storm run-off (90 days). Reinstall air vent on Well 2, install raw water sampling tap and seal well slab cracks (60 days). PWS should install locked & secured sampling taps within distribution main lines to avoid customers using spigots for personal needs (gardening, etc.). All outdoor customer spigots should be equipped with hose bib anti-siphon vacuum breaker relief device, to prevent backflow. 04-23-2021
