SO REC 4 -Between 1:10,000 and 1:15,000 Minor Deficiency #1: Failure to post the correct well number. Technical Basis: The state well number should be posted to provide identification consistent with State policy. Corrective Action: Change well number to read 55-595737. 07-18-2022 08-03-2022
SO REC 4 -4 Minor Deficiency #1: Failure to post the correct well number. Technical Basis: The state well number should be posted to provide identification consistent with State policy. Corrective Action: Change well number to read 55-595737. 07-18-2022 08-03-2022
FW REC 10 -PRIVATE OR PUBLIC Deficiency #7: Unable to determine water level in hydro-pneumatic tank. Technical Basis: Per engineering Bulletin No. 10, Chapter 5, D.4. LEVEL GAGE AND CONTROLS. All hydro-pneumatic tanks shall have a water level indicator. Corrective Action: Replace the sight glass. The sight glass should be insulated allowing protection from the sun and freezing during cold weather conditions. 07-18-2022 08-03-2022
DS REC Deficiency #8: Unknown wire/cable in plastic conduit is exposed. Technical Basis: Electrical appurtenances shall conform to the Uniform Electrical Code standards. Corrective Action: Couple ends of plastic pipe by means and methods that meet the Uniform Electrical Code standard. 07-18-2022 08-11-2022
FW MIN Deficiency #6: Unable to read pressure gage on hydro-pneumatic tank. Technical Basis: Per engineering Bulletin No. 10, Chapter 5, D.1. PRESSURE GAGES. The hydro-pneumatic tank shall be equipped with easily visible pressure gages showing instantaneous pressure in the tank. Corrective Action: Install a new pressure gage. 07-18-2022 07-26-2022
SO REC Minor Deficiency #2: Failure to maintain a clean well site. Technical Basis: Snakes, rodents and insects that dwell in debris and excess vegetation can carry contaminants and diseases that could seep into a well during precipitation events. Corrective Action: Remove all excess vegetation and all non-water related material stored within the well site. 07-18-2022 08-03-2022
FW MIN Deficiency #3: Unable to inspect storage tank drain valve due to excess vegetation. Technical Basis: Per engineering Bulletin No. 10, Chapter 6, E.4. DRAINS. Storage units, which float on the distribution system, shall be designed so they can be isolated and drained without causing an adverse loss of pressure in the distribution system. Corrective Action: Expose drain valve by removing all vegetation within the well site. A bottom drain valve is required for draining, cleaning, repair or maintenance. Deficiency #4: Unable to inspect storage tank overflow due to excess vegetation. Technical Basis: Per engineering Bulletin No. 10, Chapter 6, E.6 OVERFLOW. All water storage units shall have an overflow which extends down to an elevation between 12 and 24 inches above the ground surface and discharges freely. Corrective Action: Expose overflow pipe by removing all vegetation within the well site. Ensure the overflow pipe opening is screened with number #16 mesh to prevent undesirable material, insects, or small animals from entering the tank. 07-18-2022 08-03-2022
