OT SIG UV System has been by-passed; broken sample part on well location; operation and maintenance procedures not followed; 7/13/16L Routine monthly samples at 001 and 002 produced two positives. 7/15/16 repeat samples produced two positives at 001 and 002(note: a second sample at epds002 taken minutes apart from each other, one sample was positive and the other sample came back negative, thus causing us to strongly believe that a sampling error on the part of an operator had occurred.) 7/18/16 additional sample at 002 came back positive. investigation of system showed the eastbound UV unit was valved off and was by-passed that we believed created a dead zone thus creating a potential and favorable spot for bacterial growth. The re-sampling of system failed to obtain a well sample due to a broken sample port on the well. 07-19-2016 09-01-2020
