OT SIG ISSUES IDENTIFIED: Storage Tanks (Improper maintenance practices) ISSUE DESCRIPTION: "The interior of the 300,000 gallon storage tank was re-coated by Southwest Tank & Steel on April 9, 2021. The interior of the tank was disinfected on April 12 and refilled with water. A bacteria sample was taken on April 13 with absent results (pass). The tank was put back in service on April 14. The operator noticed a small lead around the outside of the tank. The contractor returned to the site on July 19 to make repairs to the tank floor. The tank floor was flushed, re-filled, and put back in service on July 20. No bacteria sample was taken." " 07-20-2021 08-07-2021
