OT SIG PWS WAS using 2 unapproved sources. A sanitary Survey was conducted on 4/13/2018. At that time the permit to operate was suspended and the PWS was changed to inactive until they received AOCs for the new wells and components. The PWS was in MCESD Enforcement Program until all violations were in compliance. The PWS returned to active and compliance after AOCs were issued in 2019.
